Virtual Care Consent

You'll be asked to agree to this when you complete your intake. It explains what virtual care is at Basal, what it can't do, and what you're agreeing to. It is written in plain language on purpose.
What I understand about virtual care at Basal
- My care will be provided mostly asynchronously: I answer structured questions and my nurse practitioner reviews them and responds through secure messaging, usually within one business day. There may be no live appointment unless my clinician decides one is needed.
- Virtual care has limits. My clinician cannot examine me physically. If a physical exam, in-person test, or urgent assessment is needed, I will be told to see a clinician in person or go to an emergency department, and I agree to do so.
- Prescribing is my clinician's decision. Paying for the service, or passing the online eligibility check, does not guarantee a prescription.
- Basal does not sell or ship medication. If a prescription is written, it is sent to the pharmacy I choose and I buy the medication there.
- I must be physically in Ontario when I receive care, and I will tell my clinician if I travel or move.
Identity
I will provide my legal name, date of birth, Ontario health card number, and government photo identification, and I understand my clinician may ask for a brief video call to confirm who I am.
Privacy and electronic communication
- My health information is kept in a clinical records system with data residency in Canada, and is handled as described in the Privacy Policy.
- Secure messaging inside my patient account is the way to communicate with my clinician. Ordinary email and text messages are not fully secure; if I choose to use them, I accept that risk.
- Prescriptions and laboratory requisitions are sent by fax or secure electronic transfer to the pharmacy and laboratory I choose.
Emergencies
Messaging is not monitored around the clock. In an emergency I will call 911 or go to the nearest emergency department, and I will tell my clinician afterward.
My rights
- I can withdraw this consent and stop care at any time, in writing. My clinician will give me a plan for continuing care elsewhere.
- I can ask for a copy of my record.
- I can ask questions about my care at any time through secure messaging.
Agreement
By checking the consent box in my intake, I confirm that I have read and understood this document, that my questions have been answered, and that I consent to receive care from a Basal nurse practitioner by virtual means as described above.
